Mandatory Online Participation Forms

9th-12th grade students must fill out all forms and upload a physical.

4th-8th grade students must upload a physical and complete the Athletic Participation Form.

1. To access the required online forms click on the “Electronic Participation Forms” tab.

2. Click on the form name, read and complete the information requested.

3. To sign the documents, click inside the signature box and hold your mouse over the box. This will allow you to create an electronic signature. If you make a mistake and need to start over, click on the refresh icon next to the signature box.

4. Upon completion of each document and saving, you will have the opportunity to save for your records.

5. Once all of the electronic forms are completed, move your cursor over to the “Download and Print” tab and click. Here is where you will download and print the Physical form to take to your physician.
